Free subscription to Style Ideas magazine with survey

In Better Homes and Gardens April 2002 issue there is an insert with survey to be completed and mailed for free subscription to Style Ideas magazine which is a decorating magazine put out by Home Depot.

I'm going to move this over to the Internet & Retail Deals forum since you have to buy or subscribe to Better Homes and Gardens in order to get that survey card....

I bought this magazine and there is no such survey in it. Perhaps this is a regional thing?

Maybe it is only for subscriptions. It was a very noticeable insert with perforated edge to be torn out.
