anybody get the rebate form for the black torchiere lamp from staples????

all the sights are showing "technical difficulty", and i hate to order before i have the form in hand. help!!! (i have a fax if somebody has one in hand.)

i tried to fill this but could not.i used the live chat from staples website CSR gave me a phone no to call and said they would mail me a rebate copy
