I have the $30 Johnson & Johnson Acuvue rebate form which has been floating around for some time. One condition of the deal is getting an eye exam from "a participating Eye Care Professional". Is that a select group or does any doctor prescribing Acuvue lenses qualify? "participating" ECP really only means one that has access to the rebate forms & can give you one. Your rebate should not be declined because of who your doctor is, I dont think the rebate fulfillment center has access to an active ECP list. In your post you stated you've had your rebate "floating around for some time," please also read in the fine print that your product purchase must have been made with in 90 days of your eye exam. The $30 rebates have been around for several years so make sure your rebate is the current one for 2003. You can tell if it is current as it will have an expiration of 12/31/03. Cinamin27....thanks for your thorough reply. In fact, I do have the current coupon and my exam was in the past 30 days, so I should be fine in that regard. The coupon, however, was printed off the Internet. I hope I don't get hung up there and that I don't need some "official" pre-printed form. |

you will need an original certificate provided by an ECP or the place you purchased your annual supply of lenses from. acuvue.com does not offer an offical certificate that can be printed from the website. [Only registered and activated users can see links. Either login above or Register Now] step #2 gives this info: Complete the $30 Rebate certificate provided by your Eye Care Professional or your place of purchase also in the terms & conditions from the above link it states "Photocopy of certificate not valid." so the printed copy you have from an internet site will probably not work either. and the certificate itself specifically states "Complete this original certificate" maybe you can check back with your ECP or where you bought the lenses ?? good luck! ![]() __________________ [Only registered and activated users can see links.
