The last question says:

"Would you be willing to fill out a short follow-up questionnaire after trying Puroast for a free 14 oz. bag of coffee?"

I think this means that first you get the 2 oz. sample and then after you answer the follow-up survey, you'll receive a 14 ounce bag. Great deal... thanks!
