I hadn't seen this one when it was first posted, but it looked familiar so I emailed them.

Just got a very fast reply from Sheila - they are OUT of tshirts so don't bother to fill out the form unless you want/need spam email from them. She said that the form was only up to collect customer info (there is a disclaimer right on the form).
