This says to fill this out for more info. It doesn't even require an address. At the bottom of the form it does have the standard big company quote "Would you like to receive valuable offers, samples
and communications including e-mails and newsletters from...". I've never actually received a sample from a form with this statement. Has anyone else? I have received samples from these companies, but always from a form that requires an address and states "you will receive a sample of (the item)".
