GRAND PRIZE: (1) A Spa Revitalizer package for two (2) to a luxury hotel/spa in Arizona. Trip includes round-trip coach airfare for two from major airport near winner's home to Phoenix, AZ, 3 nights/ 4 days hotel accommodations (one double occupancy room and room tax only), daily breakfast, lunch and dinner (excluding alcohol and gratuities), daily use of spa facilities, and 2 one hour spa or salon services per person per day. All other fees and expenses (including, but not limited to, ground transportation and taxes) are the sole responsibility of the Grand Prize winner. Travel companion must be 18 or older. If winner or travel companion is a minor under applicable state law, the other party must be his/her parent or legal guardian. Winner must take the trip no later than one year from the date that the fully completed and signed affidavits and releases are received by Sponsors. Approximate Retail Value (ARV): $ 4,000.
