All Persons who, at any time from February 5, 1996 to October 15, 2000, paid MCI's "Non-Subscriber" rates and surcharges for Direct-Dialed domestic and/or international long distance telephone calls.
To submit a claim, please click here. The deadline for submitting a claim is April 30, 2001. By submitting an Option A claim, you can receive a refund of all non-subscriber surcharges you paid, plus 50 percent of the non-subscriber rate-based charges, minus any credits you have already received for these billings. If you cannot or do not wish to provide the information needed for an Option A claim, you can submit an Option B claim and receive a flat refund of $75.

Do you declare, under penalty of perjury, that (a) you were an MCI subscriber at some time during the period from February 5, 1996 to October 15, 2000; (b) you believe in good faith that you paid MCI's Non-Subscriber Rates And Surcharges during that period; (c) these rates and surcharges were not the result of your making Dial-Around calls on MCI's network; and (d) you have not already received credits from MCI or your local telephone company for the Non-Subscriber Rates And Surcharges."
